Good Faith Estimate

The No Surprises Act provides certain protections related to health care costs and unexpected medical bills. For individuals who are uninsured or who are not using health insurance to pay for services, these protections include the right to receive a Good Faith Estimate of expected charges.

A Good Faith Estimate outlines the anticipated cost of the health care services included in the estimate. You may request a Good Faith Estimate before scheduling services or at any time by contacting Berkley Wellness.

Berkley Wellness is a self-pay practice. We do not bill insurance, submit insurance claims, or provide superbills. Fees for services are clearly disclosed in advance, and clients independently choose the services they wish to schedule.

Payment receipts are available through the Client Portal. Berkley Wellness does not participate in the insurance reimbursement process and does not guarantee coverage or reimbursement by any health plan.

Receiving a Good Faith Estimate does not require you to schedule services, continue treatment, or receive any particular number or frequency of services. You are encouraged to retain a copy of any Good Faith Estimate you receive for your records.

Additional No Surprises Act Protections

The No Surprises Act also provides federal protections against certain unexpected medical bills, including qualifying emergency services and certain services provided by out-of-network providers at in-network facilities.

You are not required to give up your protections against surprise billing and may choose a provider or facility that participates with your health plan when applicable.
