No Surprises Act

Under the federal No Surprises Act, individuals who are uninsured or who are not using health insurance to pay for services have the right to receive a Good Faith Estimate of expected charges.

Berkley Wellness is a private-pay practice, and fees for all services are disclosed prior to scheduling.

Good Faith Estimates are available upon request.

Your Rights Under the No Surprises Act

The No Surprises Act is a federal law designed to provide greater transparency about health care costs and protection from certain unexpected medical bills.

Good Faith Estimates

If you are uninsured or are not using health insurance to pay for services, you have the right to receive a Good Faith Estimate of expected charges for your care.

Berkley Wellness is a private-pay practice. We do not bill insurance, submit insurance claims, or provide superbills. Fees for all services are disclosed prior to scheduling, and clients independently choose the services they wish to schedule.

A Good Faith Estimate provides information about the expected charges for services. You may request a Good Faith Estimate at any time by contacting Berkley Wellness.

Receiving a Good Faith Estimate does not obligate you to schedule services, continue treatment, or receive any particular number or frequency of services.

If Your Bill Is Higher Than Your Estimate

If you receive a bill that is $400 or more above the amount listed on your Good Faith Estimate, you may be eligible to use the federal Patient-Provider Dispute Resolution process.

You are encouraged to keep a copy of any Good Faith Estimate you receive for your records.
